Please visit the <a id=\"m_3827187566027908040OWA5a4ebd2c-adf7-0c4f-337c-5d637718ac29\" href=\"https:\/\/ramapo.medicatconnect.com\/medicat\/login.aspx\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=https:\/\/ramapo.medicatconnect.com\/medicat\/login.aspx&amp;source=gmail&amp;ust=1776874530015000&amp;usg=AOvVaw2Obt63DRN2ETK2Wo3tYksG\"><u>Patient Portal<\/u><\/a> for proper submission of all immunization documentation. When logging in, use your Â鶹´«Ã½ username and password. The \u201cHome\u201d tab of the Patient Portal provides detailed instructions on immunization requirements and how to properly submit your documentation.<\/p>\n<p>For questions or concerns, please reach out to the Health Services staff via the Messages tab within the Patient Portal or call 201-684-7536.<\/p>\n<p>If you are a full-time undergraduate or a part-time \/ full-time graduate student who is under the age of 31 years at the time of enrollment, you must submit:<\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Proof of two MMR vaccines, with the first dose given on or after your first birthday.<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">\u00a0Alternatively, you may submit titers showing immunity to Measles, Mumps, and Rubella\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Proof of 3 doses of Hepatitis B (not required if you are taking less than 12 credits) <\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Alternatively, you may submit a titer indicating immunity.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"font-weight: 400\">Proof of a Meningitis ACWY vaccine given at age 16 or later<\/span>\n<ul>\n<li><strong>Residential students will not be able to move into on-campus housing until proof of an up-to-date Meningitis ACWY given on or after your 16th birthday is received<\/strong><\/li>\n<li><span style=\"font-weight: 400\">If you are a commuter student aged 19 or older, you are waived from this requirement.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\">A completed and signed Tuberculosis Screening Form.\u00a0 This form is to be completed by all incoming students.<\/li>\n<li style=\"font-weight: 400\">If you are having trouble locating your immunization records, try utilizing your <a href=\"https:\/\/www.ramapo.edu\/health\/wp-content\/uploads\/sites\/383\/2024\/10\/immunizationregistry92424.pdf\">state&#8217;s immunization registry<\/a>.<\/li>\n<\/ul>\n<hr \/>\n<h3><a id=\"s2\"><\/a>Returning Student Information<\/h3>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">If you are a returning student or a readmit student and you submitted your vaccination information within the past 10 years, you do not need to submit this information again.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">You are required to submit a new <\/span>Tuberculosis Screening Form through Medicat, Â鶹´«Ã½&#8217;s secure immunization portal. Please visit the <a id=\"m_3827187566027908040OWA5a4ebd2c-adf7-0c4f-337c-5d637718ac29\" href=\"https:\/\/ramapo.medicatconnect.com\/medicat\/login.aspx\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=https:\/\/ramapo.medicatconnect.com\/medicat\/login.aspx&amp;source=gmail&amp;ust=1776874530015000&amp;usg=AOvVaw2Obt63DRN2ETK2Wo3tYksG\"><u>Patient Portal.<\/u><\/a> When logging in, use your Â鶹´«Ã½ username and password. The \u201cHome\u201d tab of the Patient Portal provides detailed instructions on immunization requirements and how to properly submit your documentation.<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Tuberculosis (TB) testing is required for students at high risk for TB no more than 6 months prior to your return to Ramapo College of New Jersey.\u00a0<\/span><\/li>\n<\/ul>\n<hr \/>\n<h3><a id=\"s3\"><\/a>Medical or Religious Exemptions<\/h3>\n<p>All requests for Religious and Medical Exemptions are due prior to the start of the semester. Submit religious or medical exemptions through the document upload tab on the Medicat <a href=\"https:\/\/ramapo.medicatconnect.com\/medicat\/login.aspx\">Patient Portal<\/a>.<\/p>\n<h4><strong>Medical:<\/strong><\/h4>\n<p>Per N.J. 8:57-6.14, students are not required to have vaccinations that are medically contraindicated based upon valid medical reasons as enumerated by the U.S. Department of Health and Human Services Advisory Committee on Immunization Practices (ACIP).\u00a0<span style=\"color: #333333\">For current ACIP immunization schedules and recommendations, including contraindications, visit the\u00a0<\/span><a href=\"https:\/\/www.cdc.gov\/acip\/vaccine-recommendations\/index.html\" target=\"_blank\" rel=\"noopener\">CDC link on the ACIP recommendations article<\/a><span style=\"color: #333333\">.<\/span><\/p>\n<p>To apply for a medical exemption from required vaccinations, your healthcare provider must complete the\u00a0<a id=\"m_-2049070215342568995OWA7bfa30cf-feda-907f-60a2-c38d145990d7\" href=\"https:\/\/health.tcnj.edu\/wp-content\/uploads\/sites\/129\/2019\/09\/NJDOH-Medical-Exemption-From-Mandatory-Immunization-Form1.pdf\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=https:\/\/health.tcnj.edu\/wp-content\/uploads\/sites\/129\/2019\/09\/NJDOH-Medical-Exemption-From-Mandatory-Immunization-Form1.pdf&amp;source=gmail&amp;ust=1775666351231000&amp;usg=AOvVaw19yhinzQSRf3jyYjb0EJTW\">Request for Medical Exemption From Mandatory Immunization Form<\/a>.<\/p>\n<p>Ramapo College may temporarily exclude a student with an exemption from vaccination from classes or from participating in a college-sponsored activities during a vaccine-preventable disease outbreak of threatened outbreak. The decision to exclude a student with a religious exemption will be made by the College in consultation with the New Jersey Commissioner of Health.<\/p>\n<p>Unvaccinated individuals are subject to campus surveillance testing requirements, exemption from vaccination is not an exemption for testing.<\/p>\n<p><strong style=\"color: #862633;font-family: Oswald, sans-serif;font-size: 24px\">Religious:<\/strong><\/p>\n<div>\n<p>Per N.J.A.C. 8:57-6.15, a student may be considered for exemption if the student provides a written statement (signed by the student or parent\/legal guardian if the student has not yet reached 18 years of age) explaining in their own words how the administration of immunizing agents conflicts with the student\u2019s bona fide religious tenets or practices. Please clearly cite the text of your religion that would be violated. A general philosophical or moral exemption to the vaccination will not be sufficient for an exemption based on religious grounds. Form letters and letters from religious leaders will not be accepted.<\/p>\n<p>Please note the State of New Jersey does not allow for a religious exemption to the required screening for tuberculosis.<\/p>\n<p>Ramapo College may temporarily exclude a student with an exemption from vaccination from classes or from participating in a college-sponsored activities during a vaccine-preventable disease outbreak of threatened outbreak.
